Donation Request

Each year, Willy Street Co-op allocates funds to contribute, donate to, or sponsor events and activities throughout the greater Madison and Middleton area communities. We ask that any organization wishing to make a request do so at least three weeks before your event.

Organizations are strongly encouraged to read our Donations Policy prior to applying. The Co-op gives directly to organizations only and refrains from fundraising for or contributing to third-party charitable efforts/teams and/or individuals. If you have questions about the donation request form or what information we’d like, please contact Media and Advocacy Coordinator gianofer fields by email or 608-512-6184.

Organization info
All organizations requesting funding may be asked for additional information in order to receive funding, such as a Federal determination of nonprofit status, a copy of the Wisconsin Certificate of Exempt Status, and/or to complete a IRS Form W9.
